The patient portal is an amazing new tool that YNHC provides to allow for our patients to more actively engage in their own healthcare. With the patient portal, you can now access your medical records, send messages to your provider, and schedule appointments. This is all done through the HIPAA compliant, web accessed patient portal hosted through E-Clinical Works.
Activating the Patient Portal
In order to access the patient portal, your account must be first activated. In order to activate your account, please call in to YNHC to verify that you have an updated HIPAA communication form reflecting your want to engage with the patient portal. We also will need a valid email address to communicate with you.
